The easiest, less horrific scenario would be a fresh start, a brain transplant into the blank body of a pony created beforehand, and made of host cells from the several tissues it’s made of: Nervous, skeletic muscle, connective tissue, bone marrow, and so on.
Either you can move the grey and most white matter from the original host along with most of the capillary and lymphatic system and insert it into the premade pony-shaped flesh golem, and just “heal” the white matter and blood vessels connecting it to the rest of the body, or use Strong AI to reprogram a blank brain and make the golem it was the host all along. It kinda works too, even if we are some decades removed from both points.
Otherwise it would include beheading (for the neck), amputation and massive grafting (for the legs), large capillar seeding (for the coat) and massive, traumatic surgery to the whole head area (bigger eyes and eyesocket, elongate the ear canal, changing maw and dental shape…) all while keeping the patient alive and been able to recover conciousness, and still having to create many parts in-vitro. Just not viable.
