Judging by the other pictures going with this one I doubt the pony in question has bladder issues. Also there is this thing called clothing. She is a mare so its not far fetched to wear a dress like Maud Pie did to cover her diaper.

Sometimes it’s necessary to wear them for incontinence. It would be better to have a wet diaper and suffer a little humiliation than to wet the seats at a restaurant or have an accident in a library.
