That wasn’t in the very first episode, that was in Lesson Zero. And she she was messing with the frosting because Mrs. Cake threw in an extra cupcake and Twilight worried about it being too close to another cupcake and getting more frosting on it. And she kept messing with them until she finally got them all equal, which happened to leave the cupcakes with barely any frosting left (and Spike covered in it).
 
Regardless, I think Twilight being lactose intolerant is silly. I mean, the simple reason is she just doesn’t like how much cheese is in a quesadilla. She does likes nachos with cheese, but maybe cheese on chips is fine while cheese on tortillas is not?
